Complaint Form

Our staff and chaplains are committed to providing the greatest service and support that we possibly can. However, if you feel that you were not given the appropriate care, adequate service, were mistreated in any way, or would rather receive support from a different chaplain than the one assigned, complete the complaint form below, email droberts@chaplainconsultants.com or phone 910-701-0306.

All complaints will receive the highest priority and be fully investigated.

Use this form to file a complaint against a Tubman Chaplain Network chaplain or staff member. A staff member will respond to you within 24 hours.

Please provide a range of times and include your time zone.

Complaint Information

Please provide the date and approximate time that the incident of which you are complaining about occurred.

Please provide as much information as possible in your statement of complaint. Describe the nature of the discussion between you and the chaplain or staff member, what the chaplain or staff member said or did that upset you, and other pertinent information that will help us investigate the complaint. A staff member will reach out to you to collect more information as needed.
